Speech Language Pathologist (SLP) Responsibilities: Evaluate and diagnose communication and swallowing disorders in patients with a range of conditions and diagnoses Develop and implement individualized treatment plans to address patient communication and swallowing needs Provide speech therapy services to patients, including individual and group sessions, as well as carry-over exercises and activities Collaborate with other healthcare professionals to ensure comprehensive care and goal achievement Document patient progress and treatment plans in accordance with facility policies and procedures Participate in patient care conferences and family meetings to discuss treatment progress and goals About the Speech Language Pathologist (SLP) Role: As a Speech Language Pathologist at Countryside Post-Acute and Rehabilitation Center, you will have the opportunity to work with a diverse patient population and make a meaningful impact on their lives.
