Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing in Group Homes in Roseville and surrounding communities. Position Overview: Provide on-site, team-oriented primary care to patients within the group home setting living communities Part-time providers care for patients weekdays; tailored to part-time desires and needs Manage a panel of patients autonomously seeing approximately 8-14 patients per/day Ability to design and manage your schedule Develop and implement care plans appropriate to patients and their desires Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers Collaborate with community partners within home care, hospice, and other resources as needed Qualifications: Graduate of NP program or PA program from an accredited school Must have and maintain a valid Minnesota License Professional clinical experience in primary care Proven ability to communicate effectively with strong verbal skills Excellent interpersonal and customer service skills Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required Demonstrated compatibility with Bluestone’s mission and practiticing philosophies Travel throughoutTwin Cities Metro required Preferred Qualifications: 2+ years of clinical experience and/or group home experience in another role Bluestone providers enjoy: Compensation Base + Quality/Productivity: $115-160k Generous and ongoing CME Offering Unlimited PTO and Eight (8) observed holidays General Benefits of: Health Insurance, Dental and Vision Insurance Company paid Life Insurance Company paid Short and Long-term Disability Health Savings Account (HSA) & Flexible Spending Account (FSA) Retirement plan with 4% matching contributions Pay Transparency $120,000 - $160,000 USD #J-18808-Ljbffr
