Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: 2x3 days Hours per Shift: Not specified Experience: Two (2) years of experience in a home health setting within the last fifteen (15) years License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: - Recent experience conducting comprehensive assessments of patients - Knowledge of the theory, principles, and practices of general professional nursing - Excellent organizational, interpersonal, written, and verbal communication skills - Ability to work effectively with multi-generational consumers - Ability to perform comfortably in a fast-paced, deadline-oriented work environment - Strong ability to work independently Description: This position involves performing initial in-home pre-evaluations of patients. Evaluations will include assessing health status, strengths, care needs, and preferences. The role guides the development of individualized long-term care service plans. Candidates will review consumer medical documentation and/or health referral forms as related to the case. Data from evaluations must be entered into electronic forms and transmitted as required. Documentation of any concerns or conflicting information encountered during evaluations is necessary. Emphasis on continuity of care is essential to reduce or eliminate fragmentation and gaps in treatment plans. Communication with consumers on healthcare options and service vendor options is a key responsibility. Initiating communication between eligibility consumers and the enrollment process is vital. Enhancing communication and collaborative relationships with interdisciplinary care team members is critical. Collecting quality review data and required documentation to support outcome measurements is needed. Identifying opportunities for health promotion and illness prevention will be part of the responsibilities. Maintaining a comprehensive working knowledge of community resources and services for the target population is required. The position meets all standards established and follows the outlined annual performance criteria. Additional duties may be assigned by management as necessary.
