Job Summary

We are seeking a Physical Therapist Home Care professional for a 13-week contract opportunity in the Chester, PA and the Spring Garden Philadelphia areas. This role is ideal for a licensed PT who enjoys working independently in a home care setting while helping patients improve mobility, strength, safety, and overall quality of life.

This Physical Therapist Home Care assignment offers a consistent weekday schedule, full-time hours, and the opportunity to support patients in their own homes across assigned service areas. Key Responsibilities

- Provide in-home physical therapy evaluations, treatments, and patient education based on individualized care plans.
- Help patients improve mobility, balance, strength, endurance, transfers, and functional independence.
- Document visits, progress, goals, and treatment outcomes accurately and in a timely manner.
- Communicate with patients, families, caregivers, and interdisciplinary care team members regarding progress and care needs.
- Promote safe home environments by identifying fall risks, mobility barriers, and appropriate therapy interventions.
- Manage a daily home care caseload while maintaining professional standards and patient-centered service.

Compensation and Benefits

- Pay rate: $55 to $56 per hour.
- Contract length: 13 weeks.
- Schedule: 8:00 AM to 4:30 PM.
- Hours: 40 hours per week.
- Assignment area: Home care coverage for zip codes 19013 and 19130.
- Location: Chester, PA.
