RN Care Manager: CKD/ESKD Field & Care Coordination
The Blue Venture Fund
Somatus is seeking an RN Care Manager (VBC) to support high‑needs CKD/ESKD populations. You will partner with nephrology practices, coordinate home visits, and educate patients while developing individualized care plans. Travel within the region is required, with a focus on reducing readmissions and improving outcomes.
