RN Case Manager

Job Description Position Summary: Provides coordinated case management services across the healthcare continuum in an acute care setting. Assesses, plans, implements, and evaluates individualized patient care plans in collaboration with a multidisciplinary team. Facilitates safe and timely discharge planning while ensuring appropriate resource utilization. Reviews medical necessity and supports compliance with regulatory and reimbursement guidelines. Serves as a clinical resource for patients, families, and healthcare providers. Promotes quality, cost-effective outcomes and continuity of care. Minimum Requirements: Graduate of an accredited nursing program Three (3) years of acute care nursing experience One (1) year in Case Management, Discharge Planning, or Utilization Review Preferred Qualifications: Three (3) years of case management experience in acute care Pediatric, Emergency Department, or Utilization Review experience CCM or ACM certification Company Description LanceSoft is rated as one of the largest staffing firms in the US by SIA.
