Primary Care Attending Physician
i4 Search Group is supporting a search for a full-time Primary Care Physician to deliver on-site care at skilled nursing and long-term care facilities in the Danbury, CT region, alongside providing telehealth primary care services. This role combines direct clinical care with quality management initiatives to optimize senior care.
Key Responsibilities
- In-Person & Virtual Care: Travel to provide on-site care at long-term care facilities; conduct virtual assessments and manage chronic/acute conditions via telehealth.
- Care Coordination: Collaborate with interdisciplinary teams and maintain accurate medical records.
- Quality Management: Support compliance audits, clinical evaluations, and regulatory adherence.
Qualifications
- MD or DO from an accredited institution.
- Extensive experience in long-term care (LTC) or skilled nursing facilities.
- Strong leadership, communication, and electronic medical record (EMR) proficiency.
- Knowledge of regulatory guidelines and quality improvement processes.
