Job Description

Home Care Coordinator - Registered Nurse
Pay Range: $90,000–$102,000

Responsibilities:

Lead assessments, care planning, and skilled nursing for home care clients
Build relationships with clients, families, and physicians
Mentor and empower direct care staff, ensuring compliance with regulations

Qualifications:

Clinical nursing expertise and home care leadership experience
Celebration of aging and ability to build trust
Strong organizational skills and regulatory knowledge
