Travel Physical Therapist - $2,488 per week

Overview Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in San Jose, California. Responsibilities Assess referred patients requiring rehabilitation services, provide direct patient care to evaluate medical condition, functional capabilities, limitations and rehabilitation potential. Establish and administer a treatment program with goals determined by the patient’s capacity and tolerance under the direction of the Physician. Set realistic and achievable goals, document and record the patient’s condition, and educate patients and families in appropriate physical therapy methods. Develop a plan of care for each physical therapy patient and provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications & Requirements Must have graduated from an accredited school. Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure. Current CPR certification. Develop a plan of care for each patient and provide skilled physical therapy services in accordance with physician orders. Ability to assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions. Collaborates with all disciplines to plan and evaluate team goals for each patient.
