Job Description

Requirements:

- Current state license as a Registered Nurse

- 1-2 years of experience as a Registered Nurse in a clinical care setting

- 2+ years of home health experience

- Homecare Homebase Software Experience

- OASIS Experience

- CHPN certification (preferred)

- Proof of current CPR certification

- Current driver's license

- Reliable transportation and insurance

Roles & Responsibilities:

- Assess patients in home health and hospice settings to identify physical, psychosocial, and environmental needs.

- Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.

- Provide ongoing education to patients and families regarding care, medications, and home safety.

- Supervise and provide clinical guidance to nursing assistants to ensure quality service.

- Implement appropriate pain management techniques and educate patients and families about pain management.

- Attend interdisciplinary team meetings and coordinate care with the healthcare team.
