This full-time position offers a competitive hourly salary range of $17 - $20 and is ideal for a motivated and experienced professional looking to make a meaningful difference in the lives of our residents. SUMMARY The primary purpose of the Social Services Director is to plan, organize, implement, evaluate, and direct the overall operation of our facility's Social Services Department, ensuring that the medically related emotional and social needs of our residents are met and maintained on an individual basis. This role is responsible for developing and implementing policies, procedures, and programs that promote the well-being and quality of life of our residents, while also ensuring compliance with federal, state, and local regulations. ESSENTIAL DUTIES AND RESPONSIBILITIES Review facility policies and procedures to ensure compliance with state and federal regulations Participate in developing facility social work policies and procedures Participate in quality assurance interdisciplinary team meetings Document progress in meeting the psychosocial needs of residents Work with the interdisciplinary team and administration to promote and protect resident rights and psychological well-being Complete social histories and psychosocial assessments for each resident Develop written, interdisciplinary plans of care for each resident Ensure therapeutic interventions to assist residents in coping with their transition and adjustment to our facility Provide support and education to residents, families, and significant others Coordinate resident discharge planning and make referrals for home care services Educate staff on the role of the social worker and psychosocial needs of residents and their families Supervise students assigned to social services and monitor their progress If you are a dedicated and compassionate professional with a passion for delivering high-quality care and making a meaningful difference in the lives of others, we encourage you to apply for this rewarding opportunity. #J-18808-Ljbffr
