Requirement description :
MUST NOTATE ALL DURING NAME CLEARS

• Board Certified/Board Eligible - REQUIRED
• Active SC license (will accept providers needing to obtain state license
• ACLS, BLS - REQUIRED
• DEA - REQUIRED
• Provider must confirm at time of name clear that they are interested in locum to perm - REQUIRED
• MUST submit to both locations to be considered - REQUIRED
• MUST be available to provide coverage of up to 6 days per month - REQUIRED
