Position Overview

The Canyons Post-Acute is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team. The LVN will provide quality nursing care to residents, administer medications and treatments, monitor residents' conditions, and support their overall health and well-being.

Responsibilities

• Provide nursing care according to resident care plans and physician orders.

• Administer medications and treatments within the LVN scope of practice.

• Monitor vital signs and report changes in residents' conditions.

• Document nursing care, medications, and treatments accurately.

• Assist with implementing and updating resident care plans.

• Communicate with residents, families, physicians, and other healthcare team members.

• Follow infection control procedures and facility policies.

• Maintain resident safety, dignity, privacy, and confidentiality.

Qualifications

• Active California LVN license.

• Graduation from an approved vocational nursing program.

• Current CPR/BLS certification.

• Previous skilled nursing facility (SNF) or long-term care experience preferred.

• Strong communication and documentation skills.

• Ability to work well with a healthcare team.

• Compassionate, reliable, and resident-focused.
