Geen ANW-diensten, wel autonomie, beleidsruimte en directe invloed op de kwaliteit van ouderenzorg in de regio. Je werkt overstijgend, bent zichtbaar als medisch expert en draagt bij aan consultatieve eerstelijnszorg, met NAH als één van de doelgroepen, samen met een sterk regionaal team. Je komt te werken in twee districten: DWO-NWN (Delft, Westland, Oostland en Nieuwe Waterweg Noord) en ZHE (Zuid-Hollandse Eilanden). Mocht je meer uren willen werken, dan is dat zeker bespreekbaar. Er staat momenteel nog een positie open in een andere regio binnen Zuid-Holland, die eventueel gecombineerd kunnen worden om zo meer uren te realiseren.## Dit ga je doen als Coördinerend Specialist OuderengeneeskundeAls Coördinerend Specialist Ouderengeneeskunde vervul je een essentiële rol in de zorg voor de bewoners met NAH problematiek. Je focus ligt op het optimaliseren van zorg op regionaal niveau, waarbij je zorgt voor goede afstemming en het juiste zorgaanbod op de juiste plaats. Je werkt overstijgend aan onze locaties en consulteert collega SO’s ter ondersteuning. In deze functie geef je medisch advies, coördineer je de zorgprocessen en fungeer je als sparringpartner voor andere zorgprofessionals in de regio.**Concreet betekent dat:*** Advisering over een integrale benadering van zorg voor bewoners binnen de regio.* Verantwoordelijkheid voor de kwaliteit en continuïteit van medische zorg in de districten DWO/NWN en ZHE.* Consulten verzorgen voor verschillende medische teams en optreden als sparringpartner bij complexe vraagstukken.* Actieve rol in de samenwerking en afstemming ketenpartners binnen en buiten de regio.* Deelname aan commissies en werkgroepen, zoals de WZD Commissie en Infectiepreventie, met ruimte voor verdere ontwikkeling van kennis en expertise.## Wat je van ons mag verwachten* Geen ANW-diensten.* Inschaling volgens de CAO VVT, FWG 80 (minimaal €8.210,19 tot maximaal €12.239,89 o.b.v. 36 uur per week).* Direct een vast contract voor 16 tot 36 uur per week.* Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.* Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.* Toegang tot o.a.
