Our mission‑driven company is seeking an experienced self‑motivated Licensed Mental Health Therapist to serve the behavioral health needs of our members. We take a value‑based approach to care, aiming to provide the right amount of care at the right time. As a key part of our Integrated Care Team, you will help create and implement a tech‑enabled psychotherapeutic care model. You will work with Psychiatric Nurse Practitioners and non‑clinical Community Guides in a multidisciplinary team, delivering care primarily via telehealth but also in the home or community as needed. Primary Responsibilities Assess and diagnose behavioral health conditions, both chronic and acute. Formulate treatment plans in consultation with team members as the member’s condition warrants. Ensure a behavioral health perspective is incorporated into comprehensive care planning for complex patients. Complete initial intakes, assess patient risks and needs, deliver therapeutic interventions, and refer or navigate to community resources. Collaborate with primary care providers, psychiatrists, social workers, or other professionals to discuss treatment plans and progress. Provide psychoeducation, emphasizing mental health promotion and disease prevention. Support referrals for members needing additional specialty care and advise on treatment plans until appropriate care is engaged. Establish and foster trusting relationships, ensuring care aligns with the patient’s goals and values. Participate in multidisciplinary case conferences for holistic and appropriate care. Provide coaching and education to clinical and non‑clinical team members about supporting patients with behavioral health issues. Support post‑discharge planning to prevent readmissions. Document timely and appropriately in the EMR to manage member care and coordination with the multidisciplinary team. Contribute to evolving clinical pathways and protocols for therapeutic treatment of behavioral health conditions in Reema’s care model. Job responsibilities will grow and change as business necessitates. Experience and Skills 3+ years of outpatient experience with adults enrolled in Medicaid, Medicare, or dual‑eligible. 3+ years in assessing, planning, and managing behavioral patient care through direct clinical work. Experience with individuals who have significant mental and chemical health needs, believing that recovery is possible. 1+ year of practice in telemedicine. Experience with underserved populations facing socioeconomic barriers. Excellent working knowledge of trauma‑informed and evidence‑based psychotherapy for mental and chemical health conditions. Comfort participating in an integrated behavioral health model alongside Community Guides. Comfort with electronic medical record documentation and enthusiasm for technology that supports care. Ability to make quick and accurate clinical assessments of mental and behavioral health conditions. Ability to exercise balanced judgment in evaluating situations, making decisions, and handling difficult or confrontational situations calmly. Ability to work effectively as a team, interacting with patients, primary care providers, consulting psychiatrists, mental health specialists, and administrative/support staff. Working knowledge of Microsoft Word, Outlook, Excel, telecommunication, and video conferencing; G‑Suite preferred. Education, Qualifications & Position Requirements LCSW, LMFT, or LPC with an active license in New Jersey. Ability to cross‑license in Ohio, Florida and Virginia. 18 years of age or older. Compliance with federal, state and local regulations regarding patient confidentiality and HIPAA. Proficiency with electronic medical records and other technology‑based tools.
