Physical Therapist

We're seeking a compassionate and experienced Physical Therapist to join a dedicated rehabilitation team at a Skilled Nursing Facility in Dallastown, PA . Responsibilities Evaluate residents and develop individualized treatment plans based on their functional goals. Provide skilled physical therapy services to improve strength, mobility, balance, and independence with daily activities. Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes. Collaborate with physicians, nursing staff, therapy colleagues, and caregivers to deliver high-quality interdisciplinary care. Educate residents and families on treatment plans, home exercise programs, fall prevention, and the use of assistive devices. Complete accurate and timely electronic documentation while maintaining Medicare and regulatory compliance. Supervise Physical Therapist Assistants (PTAs) in accordance with state practice guidelines. Participate in care conferences, discharge planning, and quality improvement initiatives. Qualifications Active Pennsylvania Physical Therapist license (or eligibility to obtain) Previous Skilled Nursing Facility (SNF) or Long-Term Care experience preferred Bachelor's degree required; Doctor of Physical Therapy (DPT) preferred Strong communication, documentation, and organizational skills Ability to work collaboratively within an interdisciplinary team Compassionate, patient-centered approach to care BLS certification preferred Why Dallastown, PA?
