Job Description
We are seeking a dedicated and compassionate Full-Time Physical Therapist to provide high-quality rehabilitation services in a 45-bed, family-friendly Skilled Nursing Facility. The PT will work 6-8 hours per day and is responsible for delivering comprehensive patient care, including admission evaluations, treatment sessions, progress notes, and discharge documentation. This role is ideal for a therapist committed to improving residents' mobility, function, and overall quality of life through evidence-based therapeutic interventions.
Responsibilities

Responsibilities
- Perform comprehensive admission evaluations and develop individualized treatment plans.
- Provide daily therapeutic interventions to improve strength, mobility, endurance, and functional outcomes.
- Complete all required documentation, including progress notes and discharge summaries, in accordance with facility and regulatory standards.
- Collaborate with an interdisciplinary care team to ensure cohesive and effective treatment strategies.
- Communicate regularly with patients, families, and staff to support education and care goals.
- Maintain accurate records using Point Click Care (PCC) EMR system.
- Adhere to all state, federal, and professional practice guidelines.
