In this role, you’ll help residents regain independence and improve their quality of life through personalized therapy plans. Key Responsibilities: Conduct evaluations and create individualized treatment plans tailored to residents’ goals. Deliver engaging, evidence-based interventions to promote independence in daily living. Monitor and document resident progress, making adjustments as needed. Collaborate with a multidisciplinary team to ensure comprehensive, high-quality care. Support residents recovering from illness, injury, or disability through rehabilitation services. Maintain accurate documentation while safeguarding resident confidentiality. Foster a positive, respectful, and compassionate care environment. Qualifications: Graduate of an accredited Occupational Therapy program. Active New York Occupational Therapist license. Previous experience in long-term care or skilled nursing facilities preferred.
