On Call Week Night RN

This is a full-time, salaried, exempt position. Schedule: Monday, Tuesday, Thursday 5pm-9am (Monday and Tuesday - Asheboro service area, and Thursday - High Point service area). This role is hybrid (in-person patient visits and charting can be done from home). Responsibilities Uses clinical knowledge and critical thinking skills to triages patient/family calls and determine appropriate interventions. Completes assigned visits when working during office hours. Completes home visits as necessary to perform nursing interventions. Provides patient and/or caregiver with appropriate instructions to support safe care of the patient in the home. Accurately and timely documents nursing assessment/interventions and other patient/family contacts according to agency policy in the electronic medical record. Updates the patient plan of care as appropriate. Documentation should be complete at the end each day. Contacts appropriate medical provider as necessary to provide updates on patient status and obtain orders for changes to the plan of care. Provides update of on-call activity to clinical staff at the end of each day through an emailed report. Provides verbal report to next on-call nurse if applicable. Collaborates with appropriate personnel in the event of multiple requests for immediate attention, hospitalization of patients, request for changes in level of care, requests for transfers to Inpatient Unit or when in doubt as to proper course of action. Obtains completed consent forms and completes immediate needs assessment as needed to meet agency need. Remains easily accessible by phone and is available during period of on-call. Qualifications Graduate of accredited school of nursing – Associates Degree; BSN preferred Two years clinical nursing experience Hospital, hospice, or home health experience preferred Current NC Registered Nurse license Valid NC driver's license, reliable independent transportation, and current auto liability insurance at limits required by the agency Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
