Registered Nurse (RN) Discharge Coordinator, Blount Palliative Care & Hospice, Full-Time, Days

Job Summary

Coordinates continuity of care from acute hospitalization to a lower level of care based upon the patient's needs.

Essential Functions

• All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference.

• Facilitates and improves process change as it relates to patient flow and the overall transition of care process.

• Collaborates with physicians, NP, PA, nurses and all other members of the healthcare team to facilitate timely outpatient hospital follow up visits.

• Maximizes Medicare incentives in regard to the transition of care visit.

• Completes timely documentation with respect to needs and goals of the transition of care visit.

• Reviews identified anticipated discharges, identify barriers and relay transition of care plan to primary care providers.

• Serves as a resource for physicians, nurses and any other member of the outpatient transition of care team. 

• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

• Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred

• Experience - Two (2) years in Nursing.

• Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.

• Ability to travel throughout the region
