Position Overview
The Nurse Practitioner (NP) for the BRIDGES/Adult Transition Program will deliver thorough, age-appropriate healthcare to adolescents and young adults, including those who have survived chronic illnesses. This role involves working in various settings such as inpatient, outpatient, and preoperative environments within our organization. The NP will collaborate with fellow NPs, physicians, and members of an interdisciplinary team to provide direct clinical care and act as a clinical consultant, focusing on the specific healthcare needs of the adolescent and young adult demographic, particularly concerning pulmonary issues.
A significant aspect of this position involves conducting preoperative evaluations and planning for adult patients at our facility. This includes reviewing medical charts and participating in preoperative consultations. The NP will work with interdisciplinary teams to assess medical and age-related factors, ensuring patients are well-prepared for procedures and that care is coordinated effectively throughout the perioperative process.
Additionally, the NP will support adolescents and young adults, along with their families, as they transition from pediatric to adult healthcare systems. This entails ensuring continuity of care and assisting patients in finding suitable adult medical homes.
Primary Responsibilities- Deliver comprehensive, age-appropriate clinical care for adolescent and young adult patients, including those who are chronic illness survivors, while collaborating with interdisciplinary teams to create and manage personalized care plans with an emphasis on pulmonary complications.
- Act as a clinical consultant in both inpatient and outpatient settings for the care of adolescent and young adult patients, particularly those with complex and chronic health issues.
- Engage in preoperative assessments and planning for adult patients, including thorough chart reviews to identify pertinent medical histories, comorbidities, medications, prior procedures, and potential perioperative considerations.
- Conduct and/or participate in preoperative adult clinic visits, working alongside patients, families, surgical teams, anesthesia providers, and other specialists to ensure safe and appropriate procedural planning.
- Assist patients and families in navigating the transition to adult healthcare, promoting effective communication and coordination between pediatric and adult care teams.
- Participate in educational initiatives for patients and families, as well as in care coordination and transition-planning efforts.
- Provide education and clinical guidance to healthcare team members regarding best practices in adolescent and young adult care and healthcare transitions.
- Collaborate with interdisciplinary partners to identify obstacles to successful healthcare transitions and develop tailored strategies to meet the needs of patients and families.
- 24 hours per week, consisting of two 12-hour shifts from 7:00 a.m. to 7:30 p.m.
- Flexibility to accommodate three 8-hour shifts per week based on program and candidate requirements.
- Workdays are Monday through Friday, with no weekend, holiday, or overnight shifts required.
Master of Science in Nursing.
Experience- A minimum of 5 years of nursing experience in a pediatric inpatient environment.
- Preferred experience in caring for young adult patients.
- Experience in facilitating the transition of adolescents and young adults to adult healthcare is preferred.
- Strong skills in clinical practice, communication, collaboration, and care coordination.
- Current Nurse Practitioner licensure in Massachusetts.
- Certification as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) is required.
- Must maintain additional licensure, prescribing authority, and credentialing requirements in accordance with our organization and relevant regulatory standards.
