Occupational Therapist – Home Health
Location: Peoria, AZ and surrounding areas
Compensation: Per Visit | Earn up to $105,300+ annually based on productivity

You will provide one-on-one care in patients' homes, focusing on improving functional independence, safety, and quality of life.
Key Responsibilities- Conduct in-home evaluations, treatments, and reassessments
- Develop individualized plans of care focused on ADLs and functional independence
- Address fall risk, home safety, and adaptive strategies
- Educate patients and caregivers on safe techniques and equipment use
- Collaborate with interdisciplinary team members
- Complete timely and accurate documentation
- Active Arizona Occupational Therapist license
- CPR certification (AHA)
- Minimum 1 year of clinical experience (home health preferred)
- Strong clinical judgment and time management
- Reliable transportation, valid driver's license, and insurance
- Strong communication and ability to work independently
- Flexible, independent schedule
