Job Summary We are seeking an experienced and compassionate Physical Therapist (PT) to provide high-quality home health services to patients in their homes. The Physical Therapist will develop and implement individualized plans of care to help patients improve mobility, strength, balance, safety, and overall functional independence. Local Pay: $55.00 to $60.00/hour (W-2) Schedule: Hours: 8 hours per day / 40 hours per week Schedule: Monday–Friday, 8:00 AM–4:30 PM On-Call/Weekend Requirement: None Productivity Expectation: 30 points per week Coverage Area: Highland County, Clinton County, Fayette County. Mileage reimbursement: Yes, between patient visits from the first patient to the last patient. Patient Care Setting: Home Health Dress Code: Any professional attire/scrub color. Work Location: Wilmington, OH 45177 Key Responsibilities: Provide skilled home health PT services, including gait/balance training, strengthening, mobility improvement, and home safety interventions based on individualized plans of care. Maintain accurate clinical documentation, coordinate with the care team, monitor patient progress, and meet the 30-point weekly productivity expectation. Required Qualifications: Current and active Physical Therapist (PT) license in the State of Ohio. Minimum 1 year of Home Health Physical Therapy experience. Strong clinical assessment, treatment planning, and patient education skills. Ability to work independently and manage a patient caseload effectively. Excellent communication and interpersonal skills. Preferred Qualifications: Experience with Homecare Homebase (HCHB) EMR.
