Title: Home Health Physical Therapist (PT)

Location: Charlotte, NC
Duration: 13 Weeks
Schedule: 8-Hour Day Shift (40 Hours/Week) | Monday–Friday | 8:00 AM – 5:00 PM

Home Health Physical Therapist (PT)Department Information

• Location: Charlotte, North Carolina
• Home Health Therapy Services
• Average Productivity: Approximately 6 total points/visits per day
• Minimum Experience Required: 1 Year
• First-Time Travelers Accepted: Yes, if experienced in Home Health

Radius Rule

• Traveler's permanent residence must be greater than 50 miles from the home unit.

Patient Population & Common Diagnoses

Patients commonly present with:

• Chronic Disease Management
• Wound Care
• IV Therapy Needs
• PleurX Catheter Management
• Wound Vac Management
• Adult and Geriatric Patient Populations
• Maintenance and Long-Term Care Needs

Required Licensure & CertificationsRequired

• Active Physical Therapy License (State Licensure)
• BLS Certification
• Valid Driver's License
• Proof of Auto Insurance

• Homecare Homebase (Required – No Exceptions)
• OASIS Documentation
• Physical Therapy Assessments
• Disease Process Teaching
• Wound Care
• Wound Vac Management
• IV Therapy Knowledge
• PleurX Catheter Management
• Chronic Disease Management
• Adult and Geriatric Care
• Maintenance Notes and Documentation
• Independent Home Health Caseload Management
