Vytalize Health in Kansas City region seeks an RN Case Manager to coordinate care for patients with chronic or complex conditions. You will advocate for personalized treatment plans, deliver patient-centered care, and navigate the value-based care model alongside PCPs, NPs and care teams. This role requires strong communication, organizational skills, and the ability to work independently within a remote team. Travel is minimal (<5%) and telehealth is used to support transitions of care. #J-18808-Ljbffr Vytalize Health
