Board Certified or TRULY Board Eligible in Surgery-Plastic (must become certified within 5 years) - REQUIRED no exceptions
Valid IA license - REQUIRED
Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice- REQUIRED
DEA - REQUIRED
BLS Certification and maintain recertification - REQUIRED
Clean malpractice and license history - HIGHLY PREFERRED / PREFERRED
Does the provider require visa support - H-1B cap exempt (Note in Additional Submission Details) - REQUIRED
Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- REQUIRED
