our Quality Assurance and Performance Improvement (QAPI) program by reviewing clinical documentation, monitoring quality outcomes, identifying trends and risks, and collaborating with clinical leadership to improve patient care across our Home Health and Hospice programs.This position serves as a clinical quality resource and strategic partner to leadership, helping ensure that documentation supports the patient's condition, plan of care, services provided, regulatory requirements, and organizational standards.Rather than simply identifying problems, you'll help find solutions, strengthen clinical practice, and turn quality findings into meaningful improvements.What You'll DoClinical Quality ReviewPerform clinical record reviews and audits for Home Health and Hospice patientsEvaluate documentation for completeness, accuracy, consistency, and regulatory complianceReview clinical records for alignment between the patient's condition, assessment findings, plan of care, interventions, and outcomesIdentify documentation gaps, clinical risks, and opportunities for improvementSupport accurate and timely completion of required clinical documentationProvide clear, constructive feedback to clinicians and clinical leadersQuality Assurance & Performance ImprovementSupport the organization's QAPI program by monitoring and evaluating areas such as:HospitalizationsEmergency Department utilizationFallsInfection trendsWound outcomesMedication managementOASIS quality measuresHome Health Value-Based Purchasing (HHVBP) metricsHome Health Star RatingsHospice quality measures, including HOPE and CAHPSVisit utilizationLength of StayLive DischargesPatient complaints and grievancesAdverse events and clinical incidentsOther clinical and operational quality indicatorsPerformance Improvement & Clinical SupportIdentify trends and opportunities for clinical improvementCollaborate with clinical managers to investigate quality concerns and determine root causesHelp develop and implement Performance Improvement projectsEstablish baseline performance and monitor progress over timeAssist with corrective action plans and follow-up monitoringProvide education and coaching based on audit findings and identified trendsHelp translate regulatory requirements and quality findings into practical improvements for frontline cliniciansRegulatory Compliance & ReadinessSupport compliance with Medicare Conditions of Participation and other applicable federal and state requirementsAssist with survey readiness and ongoing regulatory complianceParticipate in preparation for state, federal, accreditation, and payer audits or reviewsSupport clinical record requests, ADRs, audits, and other quality-related reviews as neededMonitor changes in clinical regulations and quality requirements affecting Home Health and HospiceHelp ensure policies, procedures, documentation practices, and clinical workflows align with current requirementsInfection Prevention & Patient SafetySupport infection prevention and control activities across the organizationMonitor infection trends and assist with investigation and follow-upHelp identify opportunities to reduce patient safety risksSupport education related to infection prevention, adverse events, falls, wounds, and other quality concernsCollaborate with preferredExperience with clinical quality, QAPI, chart auditing, regulatory compliance, utilization review, case management, or clinical leadership preferredKnowledge of Medicare Conditions of Participation and healthcare regulatory requirementsOASIS experience preferred for candidates supporting Home Health qualityStrong clinical assessment and documentation review skillsExcellent written and verbal communication skillsStrong organizational, analytical, and problem-solving abilitiesComfortable using electronic medical records, Microsoft Office, and quality reporting systemsWhy Join
