Contract | Days | 13-Week Assignment

Key Responsibilities:

• Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
• Deliver direct patient care focused on improving mobility, strength, and functional independence
• Document patient progress, treatment sessions, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient goals and discharge planning
• Educate patients and family members on home exercise programs and safe functional techniques

Required Qualifications:

• Graduate of an accredited Physical Therapy program
• Current and valid Physical Therapist licensure in the state of Oregon (or eligibility to obtain)
• Current CPR/BLS certification
• Experience working in a skilled nursing facility (SNF) setting

Preferred Experience / Skills:

• Prior travel or contract therapy experience
• Familiarity with MDS documentation and Medicare Part A/B billing in a SNF environment
