Essential Duties As an RN in this role, you will conduct assessments, develop care plans, provide case management, and support caregivers to ensure individuals receive high-quality care. You will also play a vital role in training caregivers, auditing health records, and ensuring compliance with program requirements. Assessments & Care Plans Complete nursing assessments and coordinate all other applicable evaluations. Develop and review AFC care plans in collaboration with healthcare providers and the Program Director/Care Manager. Assist in screening individuals for program eligibility and assess home settings. Plan and implement discharges and transitions from the AFC program. On‑Site Visits & Compliance Conduct monthly on-site visits. Ensure care plans are implemented and complete progress notes after each visit. Conduct regular evaluations and annual assessments to ensure compliance. Review AFC caregiver logs at least monthly. Complete and sign the designated screening tool required for MassHealth payment. Caregiver & Staff Training Train, evaluate, and supervise AFC caregivers in conjunction with the Program Director/Care Manager. Offer specialized training for AFC caregivers on healthcare topics. Provide staff training on safe lifting, transfer, and positioning. Case Management & Clinical Support Act as a resource for caregivers, individuals, and families. Provide case management for members with complex or acute health issues. Submit semi-annual health status reports to primary care physicians. Provide on-site assessment or treatment of individuals, as needed. Assist in obtaining adaptive equipment and devices. Audits & Quality Assurance Conduct periodic audits of health care record-keeping practices and document findings. Qualifications Fully licensed RN by the Massachusetts Board of Registration in Nursing. At least 2 years of recent experience in direct care for elderly or disabled adults. Knowledgeable in Positive Behavioral Supports. Valid Massachusetts Driver’s License and access to personal transportation (travel required; mileage reimbursed). Proficiency in Microsoft applications and electronic health records. Must authorize a Criminal History Background Check (CORI) and release relevant information from agencies like DPPC and DDS. Must successfully complete pre-employment physical (within 12 months before hire). TB test (within the first 30 days of hire).
