This position will be performed remotely, providing the flexibility to manage your schedule while meeting the diverse needs of our clients. Key Responsibilities\n

  • Utilize your education and training to formulate independent psychosocial diagnoses that guide treatment for each patient.

  • Collaborate closely with clients to design individualized, patient-centered treatment plans that respect their unique backgrounds and needs.
  • \n
  • Implement evidence-based behavioral interventions such as solution-focused therapy, motivational interviewing, and behavioral activation, ensuring that each approach is tailored to the client’s specific situation.

  • Actively coordinate care with other professionals, including psychologists, primary care providers, and various service agencies, to guarantee a holistic approach to patient care.

  • Monitor treatment responses either in-person or virtually, tracking each patient's progress toward their therapeutic goals and adjusting interventions as necessary.

  • Support patients in managing psychotropic medications as prescribed, particularly focusing on adherence, side effects, and the overall effectiveness of treatments.

Patient Population / Client Focus\n

This position primarily serves patients within a hospice setting, aiming to provide emotional and psychological support during a sensitive time. You will work with individuals who are navigating life-limiting illnesses and their families, addressing issues surrounding grief, loss, and mental health challenges associated with end-of-life care.

Qualifications\n

  • A Master’s degree in Social Work from an accredited program is required.
  • \n
  • Must hold a current, valid, and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) in the State of Colorado.

  • At least one year of experience as an independently licensed therapist is required, demonstrating hands-on expertise in mental health care.

Preferred Experience\n

While only one year of experience is required, candidates with additional experience in hospice care, geriatric populations, or palliative care will be viewed favorably. Familiarity with virtual therapy platforms and digital clinical documentation is also advantageous.

Compensation and Benefits\n

The salary range for this position is competitive, offering between USD 80,000 and 100,000 annually, commensurate with experience. Additionally, you will be eligible for annual incentive bonuses that enhance your earning potential.
