Clinical Case Manager — Housing & Mental Health Advocate

DESC seeks a dedicated Case Manager to provide ongoing case management for adults with severe and persistent mental illness within a multi-disciplinary team. You will coordinate housing outcomes, access to benefits, and community resources to support recovery. Travel locally for outreach and home visits, conduct psychosocial assessments, and maintain comprehensive records.
