Responsibilities

• Assess and treat patients with movement or functional limitations due to injuries or illnesses
• Develop and implement individualized physical therapy treatment plans in collaboration with the rehabilitation team; adjust plans based on patient progress
• Document initial evaluations, treatment plans, daily progress notes, and discharge summaries; communicate with physicians, nursing, and other team members
• Educate patients and families on safe exercise techniques, mobility training, and home exercise programs; address safety and transfer techniques
• Participate in discharge planning and coordinate care with other providers to ensure appropriate follow-up and referrals
• Manage travel between facilities within the assigned region and maintain adherence to the schedule and patient-care standards

Requirements

• Active Montana PT license
• Graduation from an accredited physical therapy program (bachelor, master, or doctorate)
• Basic Life Support (BLS) certification
• Experience using electronic medical records or healthcare computer applications
• Ability to perform all physical requirements of physical therapy practice (lifting, transferring, mobility tasks)
• Ability to work in a rural healthcare setting and travel between facilities
• Optional certifications such as dry needling or pelvic floor therapy may be considered
