Licensed Practical Nurse (LPN)

Overview Transitions At Home is currently seeking to hire a Full-Time Licensed Practical/Vocational Nurse (LPN) in the Rice Lake area. About the Role As an LPN, you will provide skilled nursing care including routine visits, medication management, treatments, and coordination of services. You will counsel patients and families in their place of residence under the direction and supervision of the RN Clinical Manager.
