Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve.Position Description: The new physician will see patients in skilled nursing facilities and other long-term care settings. Providers work autonomously in the field and are remotely supported by a robust clinical and administrative team. A low volume of patients are seen per day, allowing ample time to focus on all needs and to provide proper attention and support. The duties of the role may include:Provide high-quality complex geriatric careEnsure engagement with facility leadership, other providers, patients, and familiesSupervise nurse practitioners and/or physician assistantsAudit documentation for each supervised provider on a monthly basisCollaborate with other healthcare providers as neededParticipate in the on-call scheduleAttend trainings and meetings as requiredThe new physician will also focus on quality outcomes including, but not limited to:Care path compliancePrevention of avoidable transfersDecrease in polypharmacyCosts of careThe practice uses an EHR on a mobile-based platform.Requirements: Kentucky medical license DEA Board Certification, relevant field (Internal / Family Medicine, Geriatrics, etc.) Experience supervising APPs 3+ years relevant clinical practice experienceTo apply for this position, please send your resume to Jenn Kunkel at View email address on click.appcast.io or call View phone number on click.appcast.io.
