What You'll Do Be a key player in ensuring our residents receive holistic, person‑centered care Lead efforts to address the psychological, social, and physical needs of residents and their loved ones Collaborate with nursing, therapy, and administrative teams to create a true community of care Complete social assessments and gather important information at admission Maintain ongoing communication and support with residents' families Assist our Admissions Coordinator by giving thoughtful, welcoming tours to prospective residents and families Complete pre‑admission assessments to help residents transition smoothly into our facility What You Bring A degree from an accredited school of social work Licensure in the state of practice (LSW, LCSW, or LICSW required) 2+ years of experience in long‑term care preferred A heart for seniors and a passion for teamwork, problem‑solving, and compassionate care Why Join AdviniaCare?
