Physician, Geriatrics

Overview Academic Geriatrician sought by the General Internal Medicine section at Dartmouth-Hitchcock Medical Center to join the team and provide support at Nursing Home and Assisted Living facilities. Service locations include Hanover, NH; Lebanon and Claremont, NH; and Windsor, VT. The facilities offer a continuum of care, including independent living, assisted living, short-term rehabilitation, skilled nursing, memory care and Alzheimer’s care, respite care, and end-of-life care. Responsibilities Provide geriatrics care and support at nursing home and assisted living facilities within the service area. Collaborate with the General Internal Medicine team to deliver patient-centered care across the care continuum described above. Contribute to maintaining and developing an established patient base within the academic health system. Qualifications Board Certified/Board Eligible in Internal Medicine or Family Medicine. Fellowship trained in geriatrics is a plus but not a requirement. Must obtain and maintain medical licenses in the states of NH and VT.
