Oncology Nurse Navigator

The Oncology Nurse Navigator provides expert individualized care coordination to patients, and their families ensuring a timely schedule for diagnostic work-up, results reporting and communication between the patient and the multidisciplinary team ensuring the patient receives necessary referrals and follow-up. The Oncology Nurse Navigator serves as an advocate for the patient, promoting timely, competent, compassionate care and enhances the physician/patient relationship improving the experience of the patient with cancer.

Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. Collaborates and continuously partners with chronically ill or high risk patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. Evaluates the needs of cancer patients and educate them about their cancer diagnosis. Prepares patients and caregivers for what to expect throughout the cancer care continuum. Creates and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist and family/caregiver(s). Provides clinical support to Patient Navigators and Sidney Kimmel Cancer Center (SKCC) patients. Educate patients and families about disease process and potential side effects. Assist patient with treatment decision making; develop and use decision aids as appropriate. Attend and support Tumor Boards. Reduce emergency room utilization and hospital readmissions through outpatient symptom management. Follow patients throughout the care continuum including inpatient admissions in collaboration with inpatient care management resources. Support transitions of care and coordination between SKCC sites. Provide clinical consultation to the Patient Navigators. Educate multidisciplinary colleagues about the role of the Nurse and Patient Navigators, identified barriers, available resources and the navigation process. Support program growth and patient experience in all patient interactions.

Minimum Education and Experience Requirements: Education: Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferred. AND Experience: Three (3) years of Healthcare/Medical, experience in Oncology/Cancer Care preferred.

Salary Range: The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.

Positions posted as annual salaries reflect an estimated amount for a 1.0 Full-Time Equivalent (FTE) (i.e. 40 hour equivalent)), positions for less than 1.0 FTE will be prorated.
