Primary Care Physician

Join a Growing Primary Care Team Focused on Value-Based Care RoKo Staffing Consulting is partnering with a well-established primary care practice in Sanford, Florida, to recruit an experienced Primary Care Physician (MD/DO) to join its patient-centered team. We are seeking an Area of Critical Need (ACN) physician with extensive experience caring for the Medicare Advantage population and a passion for delivering high-quality, value-based healthcare. The ideal candidate is committed to improving patient outcomes through preventive care, chronic disease management, and evidence-based practices while excelling in a collaborative, team-oriented environment. Responsibilities Provide comprehensive primary care services to an established Medicare Advantage patient population. Deliver high-quality, patient-centered care using a value-based care model. Manage chronic conditions while emphasizing preventive care and improved health outcomes. Conduct Annual Wellness Visits (AWVs) and comprehensive patient assessments. Accurately document diagnoses and chronic conditions to support Medicare Risk Adjustment (MRA) and RAF score optimization. Monitor and improve HEDIS quality measures and other clinical performance metrics. Collaborate with interdisciplinary care teams to coordinate treatment plans and enhance the patient experience. Maintain timely and accurate documentation within the electronic medical record (EMR). Qualifications MD or DO with an active Florida medical license or eligibility for licensure. Eligible to practice under Florida's Area of Critical Need (ACN) program. Board Certified or Board Eligible in Family Medicine or Internal Medicine. Demonstrated experience managing Medicare Advantage patients.
