Job Requirements

• Active Registered Nurse (RN) license
• Associate degree in Nursing required
• At least 1 year of nursing experience required
• Previous home health nursing experience required
• Ability to participate in on-call coverage as scheduled
• Professional, dependable, and comfortable working independently in a field-based environment
• Preferred clinical skills: Pleurex care, central line care, Foley and suprapubic insertion, wound care, wound vacs, and venipuncture
• Ability to follow care plans, document accurately, and provide patient-centered support in the home setting

Responsibilities

• Deliver skilled nursing care to patients in the home health setting
• Complete patient visits, assessments, and ongoing monitoring based on individualized care plans
• Provide education and support to patients and families to promote safe, effective home care
• Perform clinically appropriate nursing interventions within scope of practice
• Support wound care, catheter care, line care, and other specialized treatment needs as assigned
• Document patient status, interventions, and outcomes accurately and in a timely manner
• Coordinate with the interdisciplinary care team to help ensure continuity of care and positive patient outcomes
• Participate in scheduled on-call coverage and respond professionally to patient needs after hours
