Responsibilities
- Carry out delegated components of occupational therapy evaluations and treatments under supervision.
- Implement treatment plans and therapeutic activities aligned with patient goals.
- Monitor patient progress, adjust interventions as directed, and document outcomes.
- Educate patients and family members on treatment rationale, safety, and home exercise programs.
- Communicate with the interdisciplinary health care team to coordinate care and progress toward discharge.
- Maintain accurate, timely documentation of evaluations, goals, and treatment activities in the patient chart.

Requirements
- Licensure: Must hold an active license as an Occupational Therapist in Vermont and New Hampshire or an unencumbered compact license.
- Education: BS in Occupational Therapy or postgraduate Occupational Therapy Certification program.
- Certification: Must have passed the National Registration Exam.
- Experience: Home health setting experience is preferred.
- Ability to work in a home healthcare/hospice setting and participate in a travel assignment with day shifts.
