Job Description
Title: Physical Therapist I - Outpatient

Schedule:
- Shift: 9:00 AM - 5:30 PM, Monday - Friday
- Weekend rotation: 1 per month

Assignment Details:
- Contract Weeks: 143
- Guaranteed Hours: Not specified

Requirements:
- BLS - Basic Life Support - Within 1 month of hire or transfer
- PT - Physical Therapist license

Roles & Responsibilities:
- Evaluate patient function by performing formal/informal testing measures to assess skeletal, neurological, respiratory, and cardiovascular status.
- Develop and implement goal-specific treatment plans based on assessment data and patient tolerance.
- Educate patients, families, and caregivers regarding pathology, treatment options, and prognosis.
- Supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with regulatory standards.
- Document patient responses to treatment and changes in condition as required by policy and regulatory agencies.
- Participate in team conferences and interdisciplinary meetings to promote collaborative care.

Compliance / Notes:
- Successful completion of facility-specific orientation.
- Understanding of Occupational Safety and Health Administration (OSHA) requirements, including annual training.
- Ability to work in an environment where exposure to communicable diseases is possible.
