About the Role:

As a Licensed Practical Nurse (LPN) at The Mansion at Brigham, you will be responsible for providing direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident. With a focus on compassion, empathy, and excellence, you will help create a warm and welcoming environment that fosters trust and respect among our patients, families, and staff.

Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Conduct resident assessments and communicate findings to the healthcare team

• Maintain accurate and up-to-date records of patient care and treatment

• Collaborate with other healthcare professionals to ensure seamless transitions and coordination of care

• Support and educate patients and families on condition management, self-care, and independence

• Participate in quality improvement initiatives and maintenance of facility standards
