Key Responsibilities

• Provide physical therapy services to home health and hospice patients across the Clarkston, WA and Lewiston, ID service areas
• Manage an expected caseload of approximately 30 points per week
• Conduct evaluations, develop individualized treatment plans, and deliver skilled PT interventions in the home setting
• Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards
• Collaborate with interdisciplinary care teams to support patient goals and transitions of care

Required Qualifications

• Active Physical Therapist license in Washington State (PT Compact state)
• Idaho PT license or pending Idaho PT license accepted (Idaho is not a PT Compact state)
• Graduate of an accredited Physical Therapy program
• Ability to work independently in a home health or hospice environment

Preferred Experience / Skills

• Prior home health or hospice physical therapy experience
• Familiarity with home health documentation platforms and point-based productivity systems
• Strong organizational and time-management skills for managing a community-based caseload
