Job Responsibilities (summary) Evaluate patients’ functional status and physical therapy needs and assist in developing the plan of care with the physician and care team. Provide physical therapy interventions to relieve pain, restore or maintain function, and report patient responses and changes to supervising clinicians. Instruct patients, families, and caregivers regarding the plan of care, functional limitations, and progress toward goals. Complete evaluations, develop and document PT plan of care, submit progress notes per policy, and report outcomes and projected frequency of care. Professional and Compliance Notes Must practice within scope of state law and agency policies. EMR documentation expectations include timely completion of evaluations and progress notes in accordance with agency guidelines. Dress code: Royal blue scrubs as requested by the client.
