About the Position

Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Home Health RN Duration: 13 weeks Shift: Day-Time Hours per Shift: 8:00 AM - 5:00 PM, possible on-call Experience: Minimum of 2 years License: Current license to practice in the State of Montana Certifications: Valid driver’s license and appropriate auto insurance Must-Have: - BS degree or better from an accredited Physical Therapy program - Two years of previous PT experience preferred Description: This position is for a traveler Physical Therapist. The position involves evaluating and providing rehabilitation intervention in the home setting. Responsibilities include managing and supervising patient care as well as patient and family education. Key duties include evaluating and establishing a plan of care that meets patients' needs in the home environment. The Physical Therapist will coordinate care with other disciplines and seek orders for appropriate additional services. Direct patient care will be provided based on the established care plan and physician orders. The Therapist will be responsible for re-evaluating patient status and progress, modifying treatment as appropriate. Effective communication with physicians and other staff members is essential. Documentation of all skilled services provided to patients is required. The Physical Therapist will communicate with the scheduler on each day worked and maximize consistency of care when preparing schedules. Oversight of services provided by any Physical Therapist Assistants will be included in the responsibilities. Additional duties may be assigned by the supervisor.
