Take on a leading role in advancing patient care by performing comprehensive home health risk assessments for Medicare Advantage members in St. Landry County, Louisiana. As a Nurse Practitioner conducting per diem visits, youll deliver essential, patient-focused assessments in members' homes throughout Opelousas, Eunice, Port Barre, and surrounding areas. This position offers flexibility, autonomy, and the opportunity to impact patient outcomes without the demands of ongoing treatment management or prescribing.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your preferred work schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage plan members
• Review prior and current medical histories, evaluate medical conditions, and assess medication usage during each 45-minute visit
• Provide clear education regarding medical conditions and medications to patients and their families
• Identify care gaps and recommend solutions to promote improved health outcomes
• Visits may include annual assessments or specific post-discharge evaluations
• Travel to homes located throughout St. Landry County, with daily shift locations varying

Qualifications:

• Active Nurse Practitioner license in Louisiana
• Current BLS or CPR certification
• Board Certification via AANP or ANCC
• Minimum of 6 months experience as a practicing NP, home health experience preferred
• MSN from an accredited nursing program
