MSW / Social Worker - Hospice

Office Location Beavercreek, OH Coverage Area Troy, Xenia and Cincinnati Schedule and Call Expectations Monday-Friday, 8:00 AM – 5:00 PM. On-call rotation: one week of coverage about every 6–7 weeks. Overview Ohio Valley Hospice is seeking a passionate, dedicated Social Worker to join our team in Beavercreek, OH. The Social Worker provides emotional support, counseling, and resource coordination to patients facing terminal illness, playing a key role in the delivery of holistic care and ensuring patients’ psychosocial needs are met with dignity and respect. Strong skills in grief counseling and long-term emotional support. Responsibilities Assess psychosocial needs of patients and families. Evaluate home safety; assist with alternative care plans when needed. Provide counseling, crisis intervention, and emotional support. Maintain accurate and timely clinical documentation. Educate and assist with advance directives. Provide referrals and practical support for patients and families. Serve as liaison with community resources and agencies. Qualifications Graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education or a bachelor’s program in social work accredited by the Council on Social Work Education; meets personnel qualifications stated in 418.114(b)(3). Minimum of one year’s experience in health care; hospice experience preferred. Understanding of hospice philosophy and issues of death/dying. Experience in hospice care preferred. Demonstrates good verbal and written communication and organizational skills. Current licensure according to Medicare guidelines to practice in state of operation. Licensed driver with an automobile that is insured per state or organization requirements and in good working order. #J-18808-Ljbffr
