Registered Nurse - Skilled Nursing - Nights

Assess, plan, implement, and evaluate the nursing care of residents within the skilled nursing setting. Ensure continuity of care of the residents between shifts by providing direct care and supervising the care given by CNAs, LPNs, and supportive staff members. Participate in resident and family teaching/conferences. Maintain a role as a resident advocate with a focus on the facility's mission.

Core Job Responsibilities:

• Assess residents on admission and when conditions change. Document appropriately including Care Plans and Medical Record.
• Educate LPNs on PPS documentation requirements and ensure compliance.
• Assess and reassess pain using appropriate pain management techniques. Educate the resident and family regarding pain management. Review all records and ensure the pain log is complete.
• Assess the resident's condition and nursing needs; set goals; prescribe appropriate nursing action. Perform all aspects of resident care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Submit records and written documentation in a timely, clear, concise, and pertinent manner. Notify physician of changes in resident's condition and follow through until appropriate action is taken.
• Note and carry out physician's orders in a timely manner. Accompany physician on floor rounds and prepare all 30 and 60 day reviews in advance.
• Give a thorough report to oncoming shift by participating in walking rounds. Follow through timely on information given by shift report, resident, or family regarding resident concerns. Document complaints on appropriate form. Investigate and develop solutions, give feedback to resident/family. Keep leadership informed.
• Implement plan of care for the resident based on assessments and goals as established by the interdisciplinary care team. Evaluate plan of care for effectiveness and revise as indicated by the resident's response to treatment.
• Treat all residents and their families with respect and dignity. Identify and address psychosocial, cultural, ethnic, and spiritual needs of residents and their families. Interact with residents, families, co-workers, managers, and other staff in a kind, knowledgeable, and friendly manner.
• Administer medications, following the five medication rights and reducing the potential for medication errors. Identify and interpret clinical changes as they relate to medical diagnosis. Take personal responsibility and accountability for professional practice and growth for self, others, and the nursing profession. Learn new clinical skills independently. Supervise and lead other members of the healthcare team to accomplish the plan of care which has been developed. Delegate to LPNs and CNAs in a manner that contributes to the excellence of the nursing unit. Act as a preceptor/resource person and assist others in implementing nursing care. Function as a resident advocate by protecting the resident's rights. Remain calm in emergency and/or stressful situations. Comply with all applicable regulations and MVHS policies and procedures. Perform other duties as required.

Education/Experience Requirements:

• REQUIRED: Graduate of an accredited nursing program. Good interpersonal, written, and verbal communication skills.
• PREFERRED: Bachelor of Science in Nursing from an accredited nursing program or higher degree. One year experience in long term care.

Licensure/Certification Requirements:

• REQUIRED: Registered Nurse licensure in NYS. Preferred: Basic Life Support Certification.
