Are you a compassionate Licensed Practical Nurse (LPN) looking for a rewarding night shift nursing opportunity ? Heritage Wellness & Rehabilitation is seeking a dedicated LPN to provide exceptional, resident-centered care in a supportive skilled nursing and rehabilitation environment.

Our night shift nurses play an essential role in ensuring residents remain safe, comfortable, and well cared for throughout the night while providing high-quality nursing care and responding to residents' clinical needs.

Position Summary

The Licensed Practical Nurse (LPN) provides compassionate, high-quality nursing care to residents in accordance with physician orders and individualized care plans. The LPN monitors resident conditions, administers medications and treatments, communicates changes in condition to the appropriate members of the clinical team, and works collaboratively with RNs, CNAs, physicians, and other healthcare professionals.

Responsibilities

• Provide skilled nursing care according to physician orders and individualized care plans

• Monitor residents and identify changes in condition

• Administer medications and treatments as ordered

• Provide wound care and other nursing treatments within scope of practice

• Communicate changes in resident condition to the RN, physician, and appropriate members of the care team

• Respond promptly to resident needs, call lights, and emergencies

• Complete accurate and timely nursing documentation

• Assist with admissions, transfers, and discharges as needed

• Collaborate with RNs, CNAs, physicians, and the interdisciplinary team

• Provide compassionate care while promoting resident dignity, safety, and comfort

• Follow infection prevention and infection control protocols

• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives

• Maintain compliance with CMS, state, and facility regulations

• Support a positive, respectful, and team-oriented work environment

Qualifications

• Current Missouri Licensed Practical Nurse (LPN) license in good standing

• Current BLS/CPR Certification

• Skilled nursing, rehabilitation, long-term care, hospital, or post-acute care experience preferred

• Strong clinical and organizational skills

• Excellent communication and teamwork abilities

• Compassionate, dependable, and resident-focused

• Ability to work independently and effectively on the night shift
