Your Role

Were seeking a compassionate, experienced Home Health Occupational Therapist to join our high-performing team. The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient-centered care. Youll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home-based therapy.

What Youll Do

Conduct Occupational Therapy Evaluations to establish individualized plans of care
Evaluate patients functional abilities related to activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
Develop and implement personalized treatment plans to improve independence, safety, and quality of life
Provide therapeutic interventions addressing self-care, home management, cognition, and upper extremity function
Educate patients and caregivers on adaptive techniques, energy conservation, home safety, and use of adaptive equipment
Collaborate closely with physicians, nurses, physical therapists, speech therapists, and other care-team members to coordinate care
Monitor patient progress, update care plans, and maintain thorough, compliant documentation
Ensure compliance with all Medicare, CMS, and home health standards

What Were Looking For

Current Florida Occupational Therapist (OT) license (required)
Previous home health experience is strongly preferred but not required
Excellent communication, time management, and documentation skills
A compassionate, dependable, and professional attitude
